Salicylic acid, or 2-hydroxybenzoic acid, is an organic acid found naturally in the bark of the white willow tree (Salix alba) and the sweet birch tree (Betula lenta) and has been hailed since ancient times for its anti-inflammatory properties.

It belongs to a family of acids known as BHAs (β-hydroxy acids) and is typically prescribed by dermatologists for treating acne and warts due to its exfoliating, keratolytic, antiseptic and antibacterial action.



Due to its keratolytic effect:

  • Hyperkeratosis: reducing keratinocyte build-up and, in turn, preventing the occlusion of the pilosebaceous follicles.
  • Hyperpigmented and desquamative actinic keratoses.
  • Keratosis pilaris: improving the appearance, erythema and follicular papules of the back of the arms, thighs or buttocks, which are often chronic.
  • Other hyperkeratotic lesions: such as dry and cracked heels.

Due to its anti-inflammatory effect:

  • Acne: Use on individual spots, as a cleanser, or as a daily treatment.
  • Psoriasis plaques (on the body or the scalp): to reduce associated hyperkeratosis.
  • Rosacea: an adjunct to treatment as it is much better tolerated by this condition than AHAs.

Photo and chronoageing: reducing lentigines or hyperpigmentation by improving skin texture and improving the appearance of fine lines.


Due to its nature, salicylic acid can also be used for:

  • Hyperpigmented and desquamative actinic keratoses
  • Other hyperkeratotic lesions

Due to its anti-inflammatory effect:

  • Relieves swelling and redness.

Due to its exfoliating and keratolytic properties:

  • Helps remove dead skin cells from the skin’s surface.
  • Promotes cell turnover or renewal.
  • Reduces imperfections in acne-prone skin.
  • Opens and cleans out clogged skin pores.
  • Gets rid of and prevents the appearance of blackheads and other blemishes.
  • Softens and loosens dry, flaky or thickened skin making it easier to shed, thus relieving skin conditions characterised by flaking or excess skin cell growth, such as psoriasis, ichthyosis, dandruff, corns and hardness.

 Due to its antiseptic and antibacterial properties:

  • Regulates abnormal sebum secretion.
  • Prevents the proliferation of bacteria that reproduce when there is excess sebum, oil or impurities on the skin, causing inflammation and acne.
  • Prevents pore clogging, keeping them clean and thereby preventing the formation of comedones.



Acneic Process

Acne occurs when the hair follicles become blocked by a combination of oily substances — sebum or dead skin cells. It manifests as various types of skin blemishes with varying severity linked to the presence of bacteria, which cause inflammation and infection in the area.

No inflammatory lesions:

– Whiteheads or closed comedones: small build-ups of very fine, pale-colored oil. The pore is closed with dirt, oil, and dead cells trapped inside. The colour is down to the fact that they have not made contact with the air and have not been oxidized.

– Blackheads or open comedones: these are caused by keratin plugs and lipids that obstruct the follicle. Blackheads are also a type of clogged pore but have an open top that comes into contact with the air, causing the cells to oxidize.


With inflammatory lesions in the area:

– Sensitive small red blemishes: painful pink papules or lesions that protrude from the skin and are solid, raised and have a circumscribed appearance.

  • Spots or pustules: papules filled with fluids or pus that appear at the top of an inflamed area of skin when the walls of the affected pore begin to open.
  • Large lumps or nodules and cystic lesions: these are larger lesions that appear in the deeper layers of the skin and should be targeted with more specific treatments.

The 4 Phases of Acne

The first phase: starts with an increase in sebum secretion due to a hormonal maladjustment (increase in dihydrotestosterone), creating oily skin or skin with oily patches. The sebaceous glands are connected to the skin’s pores by the follicles. Through these channels, the sebum carries dead skin cells to the surface of the epidermis so that they can be shed.


The second phase: occurs when dead cells on oily skin or on an oily patch don’t fully shed and clog the hair follicle due to the build-up of sebum and dead cells.


In the third phase, the follicle has grown and becomes more clogged as sebum production continues. When this combination of sebum and dead cells comes into contact with the air, it oxidizes, causing a blackhead — a non-inflammatory lesion — to appear.


The fourth phase occurs if there is a lack of oxygen in this environment, which alters the natural balance of the bacterial flora causing the acne-linked bacterium Cutibacterium acne, which feeds on excess oil, to proliferate excessively. This pathogen releases inflammatory substances, leading to the appearance of various types of acne (papules, pustules, nodules, and cysts), which is the immune system’s response to the growth of this bacteria.

– Hormonal changes (puberty, pregnancy, menopause)

– Environmental factors (e.g., pollution)

– Stress

– Polycystic ovary syndrome (PCOS can cause excess acne on the face, upper back and even shoulders)

– Certain make-up or skincare products

– Certain contraceptives or medications

Specific benefits:



Salicylic acid is a peculiar β-hydroxy acid (BHA) which, unlike other BHAs, has acidic properties and high oil solubility.  This is the basis of its mechanism of action and effectiveness as it allows it to penetrate through the membranes of the skin cells in the outermost layer of the epidermis (the stratum corneum) and perform its function from the inside.


Once inside the cells, it breaks down the intercellular desmosomal bridges, which are junctions that attach cells to each other.


The destruction of these junctions allows the surface layer of the skin to shed and detach, which is known as the exfoliation effect. In this way, SA prevents infection caused by microorganisms, both bacteria and opportunistic fungi, and eliminates impurities.


Likewise, because it’s oil-soluble, it mixes with existing oil in the epidermis and with the sebaceous material stagnating in the follicles. When introduced into these oily areas, it has an exfoliating and shedding effect on both the skin’s surface and inside the pore, helping to decrease sebum secretion and sebum build-up. This sebum-regulating effect makes it a good adjuvant for treating acne and inflammation.


Another application of the formulation is as a sun-care product.  The aromatic benzene ring in the salicylate converts ultraviolet radiation (UVA and UVB) into longer-wave radiation that feels like heat, thereby acting as a sunscreen.


At concentrations of 0.2%, Salicylic Acid also has a heteroplastic effect and promotes the regeneration of the stratum corneum, while normalizing keratinisation.
